The Care Finder Program, funded by the Australian Government, provides targeted support to older people who require intensive, person-centred assistance to navigate the aged care system. The program focuses on individuals who face barriers engaging with My Aged Care or accessing community services independently.
Care finders offer practical, face to face support to help people understand aged care options, complete processes, and connect with services that can improve safety, independence, or wellbeing.
